Our mission here at Care Simplified is to provide individualized mental health and substance use care that is easily accessible to all.

Seeing the complexities associated with entering and maintaining mental health and substance use treatment, we developed a telehealth company that shifts its focus to the patient. The goal is to make admission as easy as possible, provide exceptional service, and offer the latest treatments. We aim to bring the office to you without any compromises found with traditional care.

Eric Dees

Adult Addiction and General Psychiatry

Eric Dees, MMS, PA-C, CAQ-Psychiatry

Growing up in a family plagued by mental health and addiction inspired Eric to seek the education to help others with similar issues. This background gives him a unique perspective to better understand the challenges patients face on a daily basis. Eric approaches every patient by listening first and builds their treatment around their unique goals.

Eric is a graduate of California State University Channel Islands and completed a Master of Medical Science (MMS) at Salus University in Philadelphia. He is a licensed Physician Assistant in the state of Montana. Eric is board-certified and earned a Certificate of Added Qualifications (CAQ) in Psychiatry through the National Commission of Physician Assistants. He was mentored by one of Delaware’s top addiction psychiatrists and has used this experience to propel his career.

Eric provides medication management services for anxiety, depression, post-traumatic stress disorder, obsessive compulsive disorder, bipolar disorder, psychotic disorders, and other mental health issues. He provides medication-assisted treatment for opioid, cannabis, alcohol, and stimulant use disorders. Eric utilizes motivational interviewing and cognitive behavioral therapy (CBT).

Amy Azih

Amy Azih, PA-C

Amy Azih, PA-C, is a board-certified Physician Associate who specializes in holistic psychiatry for children, teens, and adults. With 12 years of practicing medicine under her belt, Amy ensures patients virtually connected across Montana have access to premium mental health care and weight management.

Amy chose to pursue medicine because she has always been passionate about helping people live healthier, more balanced lives. Harnessing a broad spectrum of specialties under an integrated mental health paradigm, Amy pledges that her patients experience top-tier care, even from their own cozy corners of Montana. Amy’s academic background includes a Bachelor of Science in Nutrition from the University of Houston and a Master’s in Physician Assistant Studies from Touro University Nevada. This education is not just a testament to her credentials but also a promise.

Amy won’t just prescribe medication; she will collaborate with your therapist, intertwining evidence-based psychotherapy techniques and medication interventions to create a comprehensive plan for you. The importance of seeking professional guidance, especially in psychiatric medications, cannot be overstated. Primary Care

Sam Brooks PA-C

Samuel C. Brooks, MMS, PA-C, is a board-certified Physician Assistant with over seven years of clinical experience, including five years in Family Medicine and two years in Acute Care. He has dedicated much of his career to providing compassionate, patient-centered care in underserved and rural communities . This experience has taught him the importance of access to good quality healthcare and he looks forward to helping close this gap in Montana. He is passionate about educating his patients so that they can be part of developing the best plan for their conditions and capabilities.

Sam earned a Bachelor’s degree at Brigham Young University-Idaho in 2016 and then went on to complete a Master of Medical Science (MMS) at Salus University in Philadelphia. His training and experience have equipped him with a broad range of diagnostic and clinical skills, enabling him to effectively manage both acute and chronic conditions across diverse patient populations.

Fluent in Spanish, Samuel is committed to breaking down language barriers in healthcare and fostering trust with patients from multicultural backgrounds.

When Sam isn’t working he enjoys playing guitar and spending time with his Wife, Megan, and their five children.
